Abstract
The anterior cruciate ligament (ACL) rupture is a serious, frequent and costly injury. It is estimated that approximately 1 in 4 to 5 active athletes undergoing ACL reconstruction will suffer a second ligament injury. The risk of tearing the ACL in the contralateral knee is even higher. 50% of graft failures occur within the first 9 months after surgery. 3.5 years after ACL surgery only 44% return to competitive sport and less than 50% of the athletes return to preinjury level. Current concepts for returning to elite sports do not appear satisfactory, motivating our Sports Physiotherapy at the Swiss Federal Office of Sport Magglingen (FOSPO) to develop a multifactorial testing system in order to analyze athletes' readiness for performance and reinjury risk. This scoring system includes strength, knee function, movement quality, clinical outcome and subjective perception. The five factors are quantified separately and factored in the "Return to Competition Score". Strength and knee function are weigthed by 35% each, whereas the other factors each count for an additional 10%. Such a score enables clinicians to have a better overall grasp on key factors in return to training and competition issues in elite athletes.
