Abstract
Introduction. La vaccination est le moyen de prévention le plus efficace contre l’épidémie de grippe saisonnière. Malgré les recommandations et une active promotion, la couverture vaccinale reste désespérément basse parmi le personnel soignant et médical en milieu hospitalier. Le but de notre étude a été de tester si la visite médical d’embauche que doit effectuer tout nouveau collaborateur pourrait être un moment opportun pour promouvoir la vaccination contre la grippe auprès du nouveau personnel soignant et médical engagé par un hôpital universitaire.
Méthode. Tous les nouveaux employés devant travailler aux contacts des patients et qui ont dû effectuer une visite médicale d’embauche entre la fin de l’épidémie de grippe 2016 et le début de la campagne de grippe suivante ont été randomisés et placés soient dans un groupe contrôle ou soient dans un groupe intervention. L’intervention a consisté en un entretien semi structuré et la transmission d’une brochure informative sur le thème de la grippe et de la vaccination durant la visite médicale d’embauche et l’envoi d’une carte postale rappelant la vaccination contre la grippe au début de la campagne de vaccination contre la grippe suivante. Les taux de vaccination durant la campagne ont été comparés dans les 2 groupes.
Résultats. 357 employés ont été inclus. Les taux de vaccination se sont révélés identiques dans les 2 groupes : 79/172 (46%) dans le groupe contrôle et 92/185 (50%) dans le groupe intervention. Un taux de vaccination significativement plus élevé a été mis en évidence chez les médecins (70/117, 60%) que parmi les autres professions (101/240, 42%, p = 0.001).
Dans une analyse exploratoire prédéfinie parmi les médecins, il a été mis en évidence un taux de vaccination plus élevé dans le groupe intervention (36/51, 71%) que dans le groupe contrôle (34/65, 52%, p = 0.046).
Conclusion. La promotion de la vaccination contre la grippe durant la visite médicale d’embauche n’a pas permis d’augmenter le taux de vaccination chez les employés devant travailler aux contacts des patients durant la campagne de vaccination contre la grippe suivante. Les résultats suggèrent cependant un impact favorable sur les médecins. Il pourrait être intéressant d'utiliser des stratégies de communication adaptées aux différentes catégories de travailleurs de la santé pour promouvoir le vaccin antigrippal lors de la visite médicale d’embauche.

Background: Vaccination is the most effective prevention of seasonal influenza. Despite its recommendation and active promotion, vaccination coverage remains low among healthcare staff. The goal of the study was to test if a pre-employment health check is a good opportunity to promote future vaccination against influenza among healthcare workers newly hired by a university hospital.
Methods: All new hospital employees active at the bedside who underwent a pre-employment health check between the end of 2016’s influenza epidemic and the start of the next influenza vaccination campaign were randomly allocated to a control group or an intervention group. The intervention consisted of a semi-structured dialog and the release of an information leaflet about influenza and influenza vaccination during the check-up, and the shipment of a postcard reminder 2 weeks before the next vaccination campaign. Vaccination rates during the campaign were compared among the two groups.
Results: Three hundred fifty-seven employees were included. Vaccination rates were similar in both groups: 79/172 (46%) in the control and 92/185 (50%) in the intervention group. A significantly higher rate of vaccination was noted among physicians (70/117, 60%) than among other employees (101/240, 42%, p = 0.001). In a pre-defined exploratory analysis among physicians, the vaccination rate was higher in the intervention group (36/51, 71%) than in the control group (34/65, 52%, p = 0.046).
Conclusions: Promotion of the influenza vaccine during pre-employment health check did not improve the vaccination rate of newly hired hospital healthcare workers overall during the next influenza vaccination campaign. Results suggest a favourable impact on the vaccination rate of physicians. Thus, there may be an interest in using communication strategies tailored to the different categories of healthcare workers to promote the influenza vaccine during pre-employment health check.
