Le processus de fragilité est un processus « silencieux » et « insidieux », accepté par tous comme un authentique syndrome gériatrique avec une base biologique. Si sa physiopathologie semble distincte de celle du vieillissement, faisant ainsi de la fragilité un processus potentiellement réversible, elle est encore bien mal précisée et principalement basée sur des données issues d'études transversales. Cette revue propose une approche physiopathologique de ce processus au travers de la présentation des mécanismes intrinsèques et des facteurs activateurs potentiels de la « spirale de la fragilité ». Une considération particulière sera faite sur la contribution de l'immunosénescence et de l'inflammaging dans la physiopathologie du processus de fragilité.
The frailty process appears as a "silent" and "insidious" process commonly accepted as an authentic geriatric syndrome with a biological basis. Although its pathophysiological mechanisms seem to be distinguishable from ageing itself, thus making the frailty process potentially reversible, they are still unsatisfactorily specified because mainly based upon data of transversal studies. This review suggests a pathophysiological approach of the frailty process through the presentation of the intrinsic mechanisms and of potential activating factors of the "cycle of frailty". Particular consideration will be given to the contribution of immunosenescence and inflammaging within the pathophysiology of the frailty process.
