Abstract
La multimorbidité, ou co-occurrence de plusieurs maladies chroniques, prend une importance croissante pour les professionnels de santé et l’organisation du système de santé. Elle a d’importantes conséquences pour le patient en termes de qualité de vie et d’impact fonctionnel, pour le médecin de famille en termes de compétences d’accompagnement et d’intégration et pour le système de santé en termes de coûts et d’organisation. Dans cet article, nous introduirons les notions de conditions chroniques, de multimorbidité, ainsi que son impact (fardeau) sur le patient et le médecin de famille, l’importance d’une priorisation des soins et des compétences en santé (health literacy) du patient, les conséquences de la polymédication et l’importance d’un réseau professionnel de santé. Ces thèmes seront développés tout au long du présent numéro.
Multimorbidity, or co-occurrence of several chronic diseases, is of increasing importance for health professionals and the organization of the health care system. It is important for patients, particularly in relation to quality of life and functional status, for family practitioners in relation to support and coordination skills and for the health system in relation to costs and organization. In this article we introduce the concepts of chronic conditions, multimorbidity and its impact (burden) on the patient and the family practitioner, the importance of a prioritization of care and of the patient's health skills (health literacy), the consequences of polypharmacy and the importance of a network of health professionals. These themes will be developed throughout this issue.
