Abstract
L’avenir de la collaboration interprofessionnelle pour faire face à la multimorbidité peut être abordé à trois niveaux : l’organisation du système, la coordination des soins et la défense de la relation personnelle entre le patient et son soignant et entre soignants (médecin, pharmacien, infirmière, auxiliaire de soins, travailleur social). Le développement des systèmes innovateurs doit laisser une large place à la relation qui doit primer sur le contrôle et la maîtrise.
[The future of interprofessionnal care for multimorbid patients can be considered at 3 levels: organisation of the system, coordination of care and promotion of relationship between patients and careers and between careers (doctor, pharmacist, nurse, health and social workers).
