Primary Lung pathology without any intraluminal lesion needs extensive lung resections but It is always necessary to rule out any intraluminal pathology before submitting the patient to a Pneumonectomy. The case described illustrates how a last minute, on the table Exploratory Bronchoscopy revealed a Broncholith which avoided an extensive surgery thus reducing the morbidity.
